Abstract

Could the 1946 cholera epidemic in Korea have been avoided? Evaluating past epidemics and the responses to them is crucial for preventing future disasters. However, the assessment of the 1946 cholera outbreak in Korea and the corresponding measures has been insufficient. While many studies have analyzed the outbreak’s causes and implications from international political and social perspectives, evaluations of the public health capabilities of the U.S. military government and Korean medical personnel remain inadequate. Without a thorough understanding of these aspects, it is difficult to clearly identify the responsibilities of the U.S. military government and Korean health authorities during the 1946 cholera containment efforts, which may lead to a superficial understanding of the epidemic response. This study analyzes the public health capabilities of the U.S. military government and Korean medical personnel, focusing on medical knowledge, vaccination efforts, and quarantine measures. Specifically, it examines the understanding of the disease transmission mechanisms, direct interventions against the pathogen through vaccination, and approaches to patient and public isolation and sanitation management. Successful public health interventions require a certain level of knowledge, technology, and practical application. The study first highlights the controversies surrounding the initial diagnosis of cholera, revealing differences in perspectives and information gaps between the U.S. military government and Korean medical personnel. An analysis of medical knowledge indicates that while the U.S. authorities possessed more comprehensive knowledge of cholera than their Korean counterparts, implementing large-scale rehydration therapy, the latest medical approach at the time, was infeasible due to limitations in manpower, resources, and economic conditions. The most widely implemented public health measure was vaccination; the government rapidly produced and secured vaccines for mass inoculation. However, the differences in infection rates between vaccinated and unvaccinated individuals were minimal, and the vaccination coverage fell short of Japanese standards, indicating that vaccination efforts failed to effectively curb the cholera spread. Ultimately, quarantine remained the last viable measure to halt the spread. Comparisons between the cholera responses in Japan, the quarantine measures for Japanese individuals within Korea, and the U.S. military government’s public health efforts reveal discriminatory practices in policies targeting Koreans. Through this analysis, it becomes evident that the U.S. military government and Korean health authorities faced significant technical and resource constraints in implementing cholera containment measures. Nevertheless, failures in early containment, vaccination, and quarantine efforts are apparent. These conditions resulted in repressive public quarantine measures, which need to be understood against this background.
