Abstract
This paper examines the effect of competition among providers on the utilization of facility care services in national long-term care insurance, using district-level panel data of administrative claims from 2014 to 2021. Our findings indicate that increased competition leads to an increase in the facility care utilization ratio measured as a high fraction of beneficiaries or length of stay in facilities. However, the facility care expenditures per capita decrease with competition. The positive association between competition and facility care utilization may result not only from supplier-induced demand but also from improved service accessibility or quality in response to increased competition. Meanwhile, the negative association between competition and per capita expenditures may suggest cream-skimming behavior by suppliers. In addition, the impact of competition on facility care utilization ratio increases as the proportion of privately operated facilities and the ratio of national basic livelihood and medical aid beneficiaries increase. This suggests the possibility of supplier-induced demand in response to increased competition, particularly among basic livelihood and medical aid recipients with low or no out-of-pocket costs.
