경계선 성격조직 성향 내담자에 대한 사례연구: 전이 초점 심리치료 중심으로

본 연구는 경계선 성격 조직 성향 내담자를 대상으로 전이초점 심리치료를 적용한 사례이다. 전이초점 심리치료는 정신분석적 대상관계이론을 기반으로 내담자와 상담자간 전이분석을 통하여 경계선 성격조직의 분열된 대상관계를 통합하는 치료방법이다. 이 사례는 20대 대학생 남성으로 분열된 대상관계 특성을 갖고 있으며, 자기파괴 충동, 지속적인 가정 및 학교폭력 경험, 만성화된 아토피 피부염, 성정체성 혼란에 따른 사랑관계의 심한 방해 등 많은 이슈를 포함하고 있었다. 치료 회기는 총 61회기로, 치료중기단계에서 종결되었다. 본 연구는 먼저 내담자 기본사항, 심리검사 결과, 진단, 목표, 회기요약, 성과평가방법을 제시하였고, 전이분석과 MMPI-2 검사 결과를 기반으로 치료의 성과를 분석하였다. 그리고 경계선 성격 조직 성향을 가진 내담자에 대한 전이초점 심리치료의 주안점 및 토론 주제를 제시하였다. 마지막으로 본 연구의 한계와 제언 등이 논의되었다.
Abstract
This is a case study of applying TFP(Transference Focused Psychotherapy) for a client with the borderline personality organization. TFP(Transference Focused Psychotherapy) is a therapeutic method that integrates split object-relations of the borderline personality organization through the analysis of the transference between the client and the counselor based on psychoanalytic object-relation theory. The subject was a male university student in his twenties and has split object-relations. The case included many issues such as self-destructive impulse, domestic violence experiences, school violence experiences, chronic atopic dermatitis, sexual identity confusion. Psychotherapy was completed in 61 sessions and ended in the middle stage. Beginning with overview of case, psychological evaluation and diagnosis and summary of session and performance evaluation method were presented. The outcome of TFP(Transference Focused Psychotherapy) was analyzed based on the results of Transference Analysis and MMPI-2 test. And the main points and discussion topics of the transference focused psychotherapy for the client with the borderline personality organization were presented. Finally, the limitations and proposals of the study were discussed.
