본 연구의 목적은 우리나라 노인수발보험 제도의 평가판정체계 설계상의 방법론상 쟁점들을 보다 심도 있게 고찰하기 위해 실시되었다. 이를 위해 1차 시범사업에 적용된 평가판정체계의 주요 개념과 방법론들을 논의하고 평가판정체계의 시범사업결과를 평가해 보았다. 장기요양욕구의 평가판정체계는 대상자 선정과 자격을 결정하고, 수가와 연동되는 제도의 핵심적인 영역임에도 불구하고 평가를 통한 대상자판정기법에 대한 연구는 국내에서 전무하였다. 동시에 새로운 평가판정체계 역시 기능상태가 아닌 수발부담에 의한 등급판정 기준을 도입하면서 학계에는 생소한 개념들이 많이 소개되었다. 따라서 최근 공청회 및 서비스 제공자들이 제기하는 판정체계의 문제점은 우리나라 제도의 평가판정체계의 정확한 이해의 부재에서 비롯된 것이 많아 새롭게 생성된 개념들에 대한 고찰이 필요하였다. 연구 전반부에는 평가판정체계에서 제시하는 새로운 개념들(영역별 득점, 인정수발시간 등)에 대한 생성과정을 방법론 중심으로 논의하고 후반부에는 사회보험방식을 장기요양제도에 적용한 독일과 일본의 평가판정체계를 우리나라 체계와 비교하여 우리나라제도의 특징을 고찰하였다. 또 1차 시범사업의 자료를 활용하여 평가판정체계의 실제 적용상의 쟁점들을 살펴보았다. 평가판정체계는 현재도 수정과 보완이 지속적으로 이루어지는 연구이지만 지금까지 연구된 결과들을 바탕으로 본격적인 노인수발보험제도 실행에 앞서 장기요양제도 관련 정책 설계에 중요 사항으로 고려되어야 하겠다. 주제어: 장기요양, 평가판정체계, 타임스터디, 인정수발시간, 등급판정체계, Rasch 모형, 수형분석(의사결정나무분석, tree regression analysis)
Abstract
The aim of this study is to examine the screening system of the public long-term care policy in Korea scheduled for launching in 2008. To do so, this study analyzed the results of the studies on the development of the screening system in 2003 - 2005. Because of the complication in assessing comprehensive long-term care needs of elders, the newly-designed Korean screening system suggested several concepts about long-term care needs such as "5 dimensions of long-term care needs, care-needed time. Especially this study discusses the methodological issues in using the statistical models such as Rasch model and the tree regression analyses which were used for establishing new concepts of long-term care needs. The evaluation results showed that there are several problems in applying the research model to the actual long-term care policy. The basic logic of the Korean screening system, however, was appeared not to have serous problems in examining long-term care of elders. The findings of this study suggests that the Korean screening system should be reviewed and remodeled through the further analysis on the on-going demonstration projects.
