자폐스펙트럼장애 자녀를 둔 어머니의 응용행동분석 기반 중재 경험에 관한 내러티브 탐구

초록

현재 자폐스펙트럼장애인을 대상으로 한 ABA 기반 중재의 긍정적인 효과는 세계적으로 인정되고 있으나 한편으로는 그와 상충되는 연구 결과들도 제시되고 있다. 본 연구는 이와 같은 상이한 연구 결과들을 심도 있게 이해하며 아울러 ABA 기반 중재가 우리나라의 생태학적 체계 내에서 적용되기 위해 어떠한 면이 고려되어야 하는가를 이해하고자 탈실증주의적 접근의 연구 방법을 선택하여 접근하고자 하였다. 우리나라에서는 아직 ABA 기반 중재에 대한 공적 시행이 이루어지지 않은 점을 고려하여 현재 미국에 거주하면서 응용행동분석 기반 중재를 2년 이상 자폐스펙트럼장애 자녀에게 실시한 경험이 있는 4명의 어머니들을 대상으로 면담을 실시하였으며, 응용행동분석 기반 중재가 자폐스펙트럼장애 개인이나 가족의 삶에서 어떤 의미를 지니는지, 구체적으로 아동이나 가족의 삶의 질에 어떠한 변화가 있었는지, 또한 중재 실시의 어려운 점은 어떠한 것인지 등에 대한 이야기를 듣고자 하였다. 결론적으로 연구참여자들은 ABA 기반 중재 초기 혹은 과정 중에서 적응적 및 관계적 ‘어려움’을 직면하였으나 점차 중재를 통해 자녀, 자신, 가족의 ‘삶의 변화’를 경험하게 되었다. 연구자는 연구참여자들의 경험을 통해 ABA 기반 중재 적용의 어려움에도 불구하고 긍정적인 효과를 얻을 수 있게 된 ‘효과적인 중재 실행 요인 6가지’를 도출하였으며, 본 연구를 통해 얻게 된 주요 연구결과에 대한 논의를 통해 임상가들과 ABA 기반 중재자 및 정책입안자들을 위한 시사점들을 제시하였다.

Abstract

Applied Behavior Analysis(ABA) based programs are endorsed as a gold standard for treatment of children with Autism Spectrum Disorder(ASD) in North America. This is unusual in South Korea, where instead a non-specified eclectic approach is commonly adopted. I explored the social validity of ABA-based interventions with 4 Korean families who have lived over 6 years in the USA and experienced ABA-based programs for their own children with ASD at home, school, or regional center. Even though they had faced various difficulties while adapting to ABA based intervention, they had experienced the a lot of positive changes in their lives. I could find 6 kinds of effective factors that had an impact on the results of ABA-based intervention. I discussed these results in depth on the basis of lots of researches on ABA-based intervention on individuals with Autism Spectrum Disorder and suggested some implications for the national policies in Korea on ASD, ABA-related professions, and ABA-related profession training institutions in Korea.
