본 연구에서는 군집분석을 통하여 선행연구(배대석, 구본훈, 2006; 구본훈, 이혜린, 배대석, 2006)에서 충분히 설명되지 못한 ADHD의 K-ABC 수행의 프로파일 특성을 알아보고자 하였다. K-평균 군집분석 결과, ADHD 아동집단은 크게 세 유형의 군집으로 나누어졌으며, 각 군집은 일반 아동의 K-ABC 수행, K-PIC, 그리고 ADDES-HV 수행 특성에서 서로 비교하였다. 군집 1은 일반 아동과 비슷한 수준의 지능을 유지하고 있고, 단지 주의범위와 주의산만성, 집중력의 저하와 직접적으로 관련되어 있는 K-ABC 소검사에서만 수행이 저조하였다. 결국 군집 1은 ADHD와 관련한 증상의 정도가 그리 심하지 않고, 약물치료에 따른 반응성이 두드러지지 않고 비 약물적 치료에 보다 쉽게 순응할 수 있는 충분한 적응적 능력을 갖추고 있음이 시사되었다. 군집 2는 일반 아동이나 ADHD 아동 중 군집 1에 비해 지적 수준은 낮지만 군집 3에 비해 높은 동시처리능력과 습득도 수준을 유지하고 있는 집단이었다. 그리고 군집 2는 수회생에서 청각적 주의력 및 단기기억능력이 군집 중 가장 낮고, 운동발달 지연을 동반하지는 않았지만 언어발달이 지연되었다. 또한 이 군집은 우울증상을 보이며 사회적 관계에서의 어려움을 겪는 집단임으로 간주되었다. 군집 3은 인지처리과정척도 및 습득도척도의 표준점수에서 일반 아동 및 다른 ADHD 군집 아동들 중에 가장 낮았고, 순차처리능력 뿐만 아니라 동시처리능력에서도 현저히 저조한 집단으로 간주되었다. 이 집단은 ADHD와 관련한 기본적인 치료뿐만 아니라 환경적 요인에 대한 배려 및 교육적 노력이 강조되는 집단으로 생각된다.
Abstract
This study was carried out to investigate profile characteristics of K-ABC-K performance in children with ADHD. Using a K-means cluster analysis with several sub-scales of Information Processing Scale in K-ABC-K, we classified the ADHD group into three clusters. Then, we compared differences among these clusters in scales with K-ABC-K, K-PIC, ADDES-HV. The results indicated that 1) the first cluster had similar intelligence level to normal children, except the fact that the cluster had lower scores than the normal children in several sub-scales of attention span, distractibility, and concentration ability. It was suggested that members of the first cluster had no severe ADHD related symptoms, and had low responsiveness to stimulant medication, and had good prognoses with non-medication therapies; 2) the second cluster showed lower performance level in Sequential Processing Scale than the normal and the first cluster, and had the lowest scores in sub-scales of auditory attention and short term memory. However, the cluster showed delay in verbal development, while showing little delay in motor development. In addition, the cluster showed elevated depression level and poor social relationships; 3) the third cluster had the lowest scores in Information Processing Scale and Achievement Scale among the sub-clusters, and had lower scores in simultaneous processing scales than the second cluster. Taken together, it was suggested that the third cluster needs therapeutic approaches considering environmental factors and providing special care in educational situation beyond basic treatments.
