코로나19는 노인에게 더 치명적이며, 코로나19 확산과 그로인한 사회적 거리두기 등 대면활동 제한은 노인의 장기요양보험 인정 및 급여이용에 영향을 미칠 것으로 예측된다. 이에 본 연구는 노인을 대상으로 한 장기요양보험의 인정 및 급여이용에 있어 코로나19 확산이 어떠한 변화를 가져왔는지 살펴보고자 하였다. 이에 본 연구는 국민건강보험공단 노인장기요양보험 인정과 급여 DB에서 ’16년 1월 1일부터 ’20년 12월31일까지 인정신청자 총 6,191,430명의 데이터를 추출하여, 코로나19 확산이 장기요양보험의 인정자 수, 재가급여비, 시설급여비에 주는 영향을 단절적 시계열 분석 통해 살펴보았다. 분석결과 코로나19의 확산은 통계적으로 유의하게 장기요양보험의 인정자 수와 장기요양보험 급여비에 영향을 주고 있었다. 코로나19 확산에 따른 급여비의 변화는 시설급여와 재가급여가 다른 모습으로 나타났으며, 코로나19 확산으로 시설급여의 경우 부의 영향을 받아 감소하고 있으나, 재가급여는 정의 영향을 받아 증가하고 있음을 확인 할 수 있었다. 본 연구결과 코로나19의 확산은 노인장기요양보험의 인정자 수 및 재가급여비, 시설급여비 감소를 가져왔다. 따라서 이러한 결과는 노인 및 노인성 질환자들의 노인장기요양보험이용에 코로나19로 인한 불편함을 줄일 정책마련이 필요함을 시사한다.
Abstract
The spread of COVID-19, which is more fatal to the elderly, and restrictions on face-to-face activities, such as social distancing, are expected to affect the elderly’s recognition of long-term care insurance and the use of its benefits. Therefore, this study aimed to examine the changes this circumstance has brought about in the recognition of long-term care insurance for the elderly and the use of its benefits. To this end, this study extracted the data of a total of 6,191,430 applicants for accreditation from January 1, 2016 to December 31, 2020 from the NHIS long-term care insurance accreditation and the benefit DB for the elderly, and the effect of COVID-19 on the number of people recognized for long-term care insurance, home care benefit cost, and facility benefit cost was examined through a discontinuous time series analysis. As a result of this study, the spread of COVID-19 had a statistically significant impact on the long-term care insurance benefit cost and the number of recipients of long-term care insurance for the elderly. There was a difference between facility benefits and home care benefits due to the spread of COVID-19. Facility benefits were decreasing due to negative effects, but home care benefits were increasing under positive influence. As a result of this study, the spread of COVID-19 resulted in the decrease in the number of recipients of long-term care insurance for the elderly, home care benefits, and facility benefits. Therefore, these results suggest that it is necessary to prepare a policy to reduce the inconvenience caused by COVID-19 in the use of long-term care insurance for the elderly and geriatric patients.
