우울 아동·청소년의 공존문제유형과 인지특성 및 부모의 심리적 특성

초록

본 연구는 우울 아동·청소년이 함께 가지고 있는 공존문제유형은 무엇이며, 우울과 공존문제유형에 따라 지능으로 대표되는 인지특성이 어떠한지를 분석하고자 하였다. 또한 이들의 부모의 심리적 특성을 알아보고자 하였다. 본 연구의 대상은 서울 경기권의 8개의 임상기관에서 우울 및 종합심리검사(Full Battery)를 실시한 만7세~ 만18세까지의 아동·청소년 총 779명 중, 비우울집단 39명, 공존문제없이 우울만 보이는 단일우울집단 51명, 불안/위축을 공존문제로 보이는 우울집단 119명, 주의집중문제를 공존문제로 보이는 우울집단 42명, 불안/위축/주의집중 공존문제를 동시에 보이는 우울집단 156명 총 407명을 연구대상으로 최종 선정하였다. 또한 이들의 부 320명, 모 398명을 대상으로 선정하였다. 연구결과 첫째, 우울 아동․청소년의 공존문제유형 분포는 불안/우울(75.3%), 위축/우울(58.7%), 주의집중(54.1%) 순이었다. 둘째, 비우울집단과 우울집단으로 대별하여 지능을 살펴본 결과 우울집단의 평균이 유의미하게 낮은 것으로 나타났다. 다섯 개 연구 집단간의 인지적 특성의 차이를 분석한 결과, 불안/위축/주의집중문제공존 우울집단이 가장 인지적 특성이 저하되는 양상을 보이고 있어, 공존하는 문제 증상이 심각할수록 지능도 저하되는 것으로 나타났다. 셋째, 비우울집단과 우울집단간 부모 MMPI 척도의 차이를 살펴본 결과, 모는 건강염려증(Hs), 히스테리(Hy) 척도에서, 부는 반사회성(Pd) 척도에서 유의미한 차이가 있는 것으로 나타났다. 이러한 결과를 토대로 본 연구는 아동･청소년의 정서적 안정과 적응력을 높일 수 있도록 인격교육적 측면이나 임상연구, 상담 및 가정에서의 교육적 지도의 필요성을 시사하고 있다.

Abstract

The purpose of this study is to examine the comorbidity types of children and adolescent with depression, and to compare intelligence traits between groups with depression and those without it, based on the depression and comorbidity types. We also investigated the psychological traits of their parents. First, we included 779 children and adolescents between seven and 18 years old who took full battery assessments from eight clinical facilities. Among the patients, we finally enrolled 407 patients in this study; 39 subjects without depression, 51 subjects who had depression without comorbidity, 119 subjects who had depression with anxiety and withdrawal, 42 subjects who had depression with comorbid attention problems, and 156 subjects who had depression with comorbid anxiety, withdrawal and attention problems. Also we included their parents (320 fathers and 398 mothers) in this study. The results are as follows: first, the comorbidity types of children and adolescents with depression include anxiety/depression (75.3%), withdrawal/depression (58.7%) and attention problem (54.1%). Second, the intelligence test score is significantly lower in the depression group. Furthermore, the depression group with comorbid anxiety, withdrawal, and attention problems had the lowest intelligence traits, showing that the more severe the comorbidity symptoms, the lower the intelligence score. Third, when comparing the MMPI scale between the parents of the non-depression group and those of the depression group, there were significant difference in Hypochondriasis (Hs) and Hysteria (Hy) scales in the mothers, and Psychopathic Deviate (Pd) scale in the fathers. Based on these results, this study emphasizes the importance of character education, clinical research, counseling, and the educational guidance at home to enhance the emotional stability and adaptability of children and adolescents.
