정서장애, ADHD 및 정서장애와 ADHD 공존질환을 지닌 아동의 주의력과 인지적 특성 비교

본 연구는 정서장애, ADHD 및 정서장애와 ADHD 공존질환을 지닌 아동이 유사한 행동증상과 주의력 문제를 외현적으로 보이고 있어 감별 진단을 돕기 위해 실시하였다. 이를 위해 소아정신과 자문의 교수 및 임상심리전문가가 DSM-Ⅳ에 의거하여 진단한 초등학생들을 정서장애 집단(15명), ADHD 집단(13명), 정서장애와 ADHD 공존질환 집단(13명)으로 분류하였다. 주의력과 인지적 특성을 비교하고자 K-WISC-Ⅲ와 ADS의 점수를 일원변량분석으로 분석하였고, Sheffe 검사로 사후 검정을 수행하였다. 그 결과, K-WISC-Ⅲ의 동형찾기와 ADS에서 점수가 정서장애만 있는 집단은 정상 수준이지만 ADHD 집단과 정서장애 및 ADHD 공존질환 집단은 주의력 문제가 있는 것으로 나타났다. 이는 진단 감별을 위해 K-WISC-Ⅲ의 동형찾기와 ADS가 유용함을 시사한다. 그러나 ADS에서도 정서장애 및 ADHD 공존질환 집단과 ADHD 집단 간에는 차이가 유의미하지 않았다. 따라서 추가적인 연구를 통해 진단 감별 요인을 밝히는 것이 필요하다고 생각된다.
Abstract
The main aim of this study is to make Differential diagnosis of three groups ( a group of children with Emotional Disorders, one with ADHD and one with ADHD and Emotional Disorders) because children in these groups show similar behavioral symptoms and attention problems on the surface. For this study, professors of child psychiatric clinics and clinical psychologists classified elementary school students into three groups based on the results of DSM-Ⅳ: a group with Emotional Disorders(n=15), one with ADHD(n=13) and one with Group with ADHD and Emotional Disorders(n=13). To compare their attention level and cognitive characteristics, the scores of K-WISC-Ⅲ and ADS were analyzed by one-way analysis of variance and post hoc tests was done for Sheffe test. As a result, the group with Emotional Disorders were within normal range regarding the scores of K-WISC-Ⅲ symbol search and ADS, but the other two groups showed attention problems in the scores. K-WISC-Ⅲ symbol search and ADS are considered to be useful for Differential diagnosis. However, the difference between two groups, the group with ADHD and Emotional Disorders and one with ADHD, was not significant. For this matter, it seems to be necessary to perform additional studies to find more factors for discrimination.
