비영리 장기요양기관의 조직적 특성과 사회적 역할에 관한 고찰

본 연구는 장기요양서비스를 제공하는 비영리 장기요양기관의 조직적 특성과 사회적 역할을 고찰하는 것이다. 노인장기요양보험의 도입은 서로 다른 특성의 조직인 비영리기관과 영리기관이 합법적으로 서비스 공급 경쟁을 할 수 있는 환경을 만들어 놓았다. 무엇보다도 이윤 추구를 기관의 주요한 목적으로 삼고 있는 영리기관의 등장은 공급자간의 경쟁을 더욱 더 심화시키는 원인으로 작용하고 있다. 이윤을 남기기 위해서 부정수급, 이용자 본인부담금 할인, 노인을 기관에 유치하기 위한 호객행위 등의 다양한 형태의 불법과 편법 운영을 행하고 있기 때문이다. 본 글에서는 장기요양기관 간 시장경쟁으로 발생한 다양한 문제들에 대한 해결책으로 비영리 장기요양기관의 사회적 역할을 강조하였다. 비영리 장기요양기관은 이윤추구를 목적으로 설립된 영리 장기요양기관과 다른 조직적 특성이 존재한다. 비영리 장기요양기관은 사회적 목적, 사회적 소유, 사회적 자본을 특징으로 하는 사회적 기관이다. 사회적 기관으로서의 비영리 장기요양기관의 조직적 특성은 시장경쟁으로 인해 야기된 문제들을 상쇄시킬 수 있는 역할을 할 수 있다. 비영리 장기요양기관은 경쟁자인 영리기관을 선도하여 서비스 질의 표준을 설정할 수 있고, 수익사업이 아닌 복지서비스로서 요양서비스의 정체성을 확립하고 공공성을 확보할 수 있으며, 요양보호사와 같은 사회적 약자의 권익을 보호해줄 수 있는 통로가 될 수 있다. 본 글에서는 비영리 장기요양기관은 장기요양기관이 지향해야할 서비스 공급의 역할 모델이 되어야함을 주장한다.
Abstract
This study inquires into the organizational characteristics and the social role of nonprofit long-term care institutions. Nonprofit long-term care institutions are allowed to compete with profit ones in the long-term care markets. The two different types of organizations have known as different service providers having distinct organizational characteristics. Most of all, profit long-term care institutions intensify the market competition among service providers, focusing on their efforts to increase profits rather than to provide services for the elderly. In order to get profits, profit institutions are tempted to run their organizations illegally. This study emphasizes on the social role of nonprofit long-term care institutions to address the various problems caused by competition among long-term care service providers. Nonprofit long-term care institutions have distinguished organizational characteristics from profit ones. They are the social organizations characterizing social goal, social ownership, and social capital. Nonprofit long-term care institutions, as the social organization, are able to offset the problems raised by market competitions. They can set the standard of the care quality that leads the profit organizations, ensure the publicity of long-term care service as the welfare service not the profitable business, and protect the rights of care workers. Nonprofit long-term care organizations should become the role model that all long-term care organizations follow.
