화상장애인의 화상경험에 대한 질적 연구

A qualitative research on disabled people's experiences with burn injury

초록

본 연구는 화상장애인들이 “화상 장애 이후 어떠한 경험을 하고 있는가?”를 연구 질문으로 화상장애인 6명을 선정하여 2회에 걸친 포커스 그룹 인터뷰를 시행하였다. 연구 참여자의 실재 경험을 이야기하게 하였으며, 이러한 경험들에 대해 상호작용 맥락에서 구조가설을 도출하였고, 도출된 구조가설의 의미를 파악하고 이해하고자 하는 목적으로 수행되었다. 연구결과는 다음과 같다. 첫째, 동질적 공간에서 이질적 공간으로의 과정에서 갈등을 겪고 있었다. 둘째, 화상장애인으로써 비장애인에 의한 차별뿐만 아니라 같은 장애인들로부터의 차별 또한 매우 심각해 이중차별을 당하는 것으로 나타났다. 셋째, 사회적 죽음(소외나 배제의 단계를 넘어선)을 피하기 위하여 도구적 의사결정(의사소통과 행동에 있어서)을 하는 것으로 나타났다. 넷째, 자신에 대한 현실인식을 회피하는 것으로 나타났다. 이러한 연구결과들을 토대로첫째, 화상발생 이후 화상치료 초기에서부터 심리적, 사회복지적 접근을 할 수 있는 제도가 필요하다. 둘째, 인식개선을 위한 프로그램 개발과 시급한 시행이 필요하다. 인식개선 대상자로 비장애인뿐만 아니라 일반 주류장애인과 더불어 화상장애인 본인과 화상장애인의 가족구성원이 반드시 포함되어야 한다. 셋째, 효과적인 자기주장을 할 수 있는 프로그램이 필요하다. 즉, 화상장애인은 사회적 편견과 차별을 두려워할 뿐만 아니라 스스로 고립을 선택하는 경향이 나타나고 있는데 이에 대한 대안이 마련되어야 한다고 제언하였다.

Abstract

A visible disfigurement can have a profound psychological and social impact upon the individual concerned. Difficulties include adverse effects on body image, quality of life, self-esteem and so on. In this article, we aimed to find out what the disabled people with burn injury experience in their lives. This study analysed the interviews elicited from participants and ended up with several results. First, the disabled people with burn injury are worrying about moving from where they lived in similar situations as patients to where they have to live with people having prejudices and exclusions. Because where they have to go is not what they want. Secondly, they experience double discriminations. Burn injured people are excluded not only from ordinary people, but also disabled people with no burns in their community. Thirdly, those who are disabled with burn injury try to escape from social death(beyond exclusion and marginalization) and likely to choose instrumental decision making. They can easily abandon their intentions and rights and conform to others and social norms. Fourth, we reach the conclusion that they are apt to identify their former self-image before burns and have difficulties in adjusting to their new image which is hard to accept. So, they try to find a way out of this difficulty. According to the conclusions above, we suggest some ideas as follows : disabled people with burn injury need ongoing psychological and social systems to support their adjustment, because burns have an effect on them for entire life, not just temporarily. Next is there need a change of social perception to disabled with burn injury. Finally, we have to make a room and pathway for burn injured people to get out of their isolated place and it is necessary to make some programs, such as self-assertive program and self-expression program.
