의료개혁이라는 중요한 역사적 과제를 달성한 오바마 정부는 심각한 지지율 하락으로 고민하고 있으며 오바마 정부의 일련의 정책들에 대한 반발은 미국 내 대중적 보수주의의 물결을 크게 일으키고 있다. 개혁정책들에도 불구하고 오바마 정부의 개혁정책이 밑으로부터 미국민 사이에서 별 다른 호응을 얻지 못하고 오히려 티파티와 같은 보수적인 정치운동이 활성화되는 계기를 제공하고 함으로써 미국 의료개혁의 앞날에는 많은 불확실성이 남아 있다. 그러므로 의료개혁에 대한 이념적 대립의 쟁점을 둘러싼 미국적 맥락에 대한 검토는 중요한 의미를 지닌다. 왜냐하면 특히 미국사회에서 밑으로부터, 대중적 흐름에서 보이는 반 의료개혁적 정서나 반대여론의 확산을 설명하는 것이 쉽지 않거나 일면적인 해석에 그치기 쉽기 때문이다. 이 글은 이러한 문제의식에서 미국의 의료개혁을 둘러싼 미국 사회 내의 이념적 논의의 쟁점들을 검토하고 평가하였다. 특히 이 글은 의료개혁법 제정 이후의 경제상황과 대중의 반응을 여론조사 결과를 중심으로 살펴보고 의료개혁에 대한 미국사회 대중의 부정적 평가와 관련된 정치이념적 지형을 확인하였다. 또 이 과정에서 공공정책과 관련된 여러 쟁점들, 예컨대 국가의 역할과 재정적자, 공공서비스의 특성과 시장실패 등의 쟁점들을 검토하였다.
Abstract
The Obama Administration has been suffering from the sharp drop in approval rates. Paradoxically, the reformative policies including healthcare reform provide the nutrition for mobilizing the conservative grass root movement such as the ‘Tea Party’. Moreover, if the Republican Party takes the majority position in Congress this mid-term election in November, 2010, the healthcare reform law will meet the serious chance of the repeal. It means a century long debate for the healthcare reform is not over. Rather it is still going on after the legislation. In this regard, it is important to figure out the political context in the United States and compare the different point of view between the ideologically confronting groups; the progressive and the conservative. If not, we misunderstand and fail to explain why grass root people in the US show the anti-sentiment against the healthcare reform that targets the poor, middle class. This paper explores the economic situation and survey polls, then analyze the various topics underlying the healthcare reform debate. This study points that policy debate is exactly along the traditional line of the commanding heights, the State and the Market. As a conclusion, instead of supporting either the public or the market, this paper admits the hybrid form of the healthcare system, and emphasizes that the new institutions should be able to change the behavior of the various stake-holders via diverse incentives and penalties.
