피질 이형성증은 약물 난치성 뇌전증을 주 증상으로 하는 희귀 질환이다. 이 증례에서는 뇌전증 없이 왼쪽 상지와 하지의 운동 기능 장애 및 근위축을 호소하는 37세 여성 환자에 대해 보고하고자 한다. 뇌파 검사 및 신경근육전기진단검사 그리고 유발전위 검사에서 이상이 발견되지 않았다. 뇌 자기공명영 상 검사 결과 우측 측두엽 피질의 국소적 피질 이형성증 및 피질척수로 위축 소견을 보였다. 우리가 아 는 한, 뇌전증 없이 운동기능 장애와 근위축을 주증상으로 하는 사례는 최고의 보고이다.
Abstract
Cortical dysplasia is a rare disease with intractable epilepsy as the main symptom. Herein, we report a case of a 37-year-old woman who complained of motor dysfunction and muscle atrophy in the left upper and lower extremities, but did not have epilepsy. No abnormalities were found on electroencephalography and electromyography. Magnetic resonance imaging revealed dysplasia in the right temporal cortex and atrophy in the right corticospinal tract at the medullar and brain stem level. To the best of our knowledge, this is the best report of such a case with motor dysfunction and muscle atrophy without epilepsy.
