Abstract
We report a rare and fatal case of invasive conidiobolomycosis in a 63-year-old male patient with chronic myeloid leukemia who underwent alloge- neic hematopoietic cell transplantation. Despite receiving prophylactic antimicrobial therapy, the patient developed neutropenic fever on hospital day (HD) 16, followed by progressive respiratory symptoms, including purulent sputum and dyspnea, by HD 21. Sputum fungal cultures revealed white to light gray cottony colonies with hyaline, ribbon-like, sparsely branched hyphae. Amphotericin B was switched to isavuconazole due to sus- picion of mucormycosis. After slide culture, papillate globose conidia and zygospores were observed. MALDI-TOF MS was unable to identify the organism. The patient’s condition deteriorated rapidly, leading to upper airway obstruction that required emergency tracheostomy and extracorpo- real membrane oxygenation. Magnetic resonance imaging showed diffuse inflammatory infiltration of the buccopharyngeal fascia and submandib- ular spaces that extended into facial tissues. Definitive identification through ribosomal DNA sequencing of the internal transcribed spacer and D1/D2 regions confirmed Conidiobolus spp. infection. Liposomal amphotericin B was resumed on HD 50, but leukemia relapsed on HD 74, and he was transferred to palliative care on HD 78. This case underscores the importance of considering Conidiobolomycosis in the differential diagnosis of invasive fungal infections in immunocompromised patients, particularly those with hematologic malignancies.
