초록

This study developed an integrated gait age scale to predict fall risk and cognitive decline in older adults using dual-task step pattern analysis with a Smart Step mat system. A cross-sectional study was conducted with 165 community-dwelling older adults (mean age 74.8±6.9 years). Participants performed three levels of step pattern tasks under single and dual-task conditions. Principal component analysis extracted "fall risk factor" (42.4% variance) and "cognitive function factor" (30.1%). Gender-specific scales revealed that for males, fall risk model (R ²=.72) included step errors, fall history, TUG, and dual-task cost, while dementia risk model (R ²=.68) included word generation, calculation errors, and DTC. For females, fall risk model (R ²=.70) included step errors, fall history, TUG, and one-leg standing, while dementia risk model (R ²=.62) included word generation and calculation errors. When integrated gait age exceeded chronological age by 5+ years, sensitivity and specificity were 86.7% and 79.5% for fall prediction (AUC=.88), and 84.2% and 77.3% for cognitive impairment prediction (AUC=.85). The scale showed excellent reliability (ICC=.87-.92) and offers a practical tool for early identification of older adults at risk.
