초록

천식은 소아 연령에서 대표적인 만성비전염성질환으로, 지난 수십 년간 전 세계적으로 유병률이 급증하고 있으며, 서구화, 도시화가 중요 원인으로 알려져 있다. The International Study of Asthma and Allergies in Childhood 연구 결과, 영국, 호주, 뉴질랜드와 같이 서구화, 산업화된 영어권 국가에서 천식 유병률이 가장 높은 것으로 알려져 있다. 그러나 같은 경제 수준의 한국, 홍콩, 싱가포르와 같은 아시아에서는 상대적으로 유병률과 중증도가 낮다.1 국가 간의 사회경제적 수준과 천식 유병률 간의 건강 조사에서도, 최빈국과 선진국에서 천식 유병률이 높은 U자 형태를 보이고 있어,2 국가의 경제 수준만으로 천식 유병률을 예측하기는 어렵다. 반면 개인 수준에서 사회경제적 요인은 1차적으로 교육과 사회 보장의 박탈, 빈곤, 스트레스로 이어지는 악순환과, 2차적으로 실내외 공기 오염, 흡연, 실내 알레르겐 농도, 미숙아, 영양, 비만 등을 매개로 하여 천식 발생과 악화에 관여하는 것으로 알려져 있다.3
