본 연구는 2018년 노인 외래 본인부담정액제 개편이 노인의 의료이용에 미친 영향이 소득수준에 따라 차이를 보이는지 국민건강보험공단 노인코호트 DB의 2016~2018년 자료를 활용하여 만 65세 이상 노인 461,225명을 대상으로 분석하였다. 교호항을 추가한 다중회귀분석을 실시했다. 분석 결과, 제도 개편 이후 전체적으로 외래 이용과 보험자부담금이 증가하고 본인부담금은 감소하는 등, 노인의 의료 접근성이 향상된 것으로 나타났다. 그러나 소득수준에 따른 효과에는 차이가 있었으며, 특히 저소득층에서 본인부담률이 크게 완화된 ‘진료비 15,001~20,000원’ 구간의 외래 이용은 오히려 감소하는 현상이 관찰되었다. 즉, 본인부담률 인하만으로는 외래 이용을 충분히 유도하기 어려울 수 있다. 다만 본 연구는 의원 외래 이용에 한정되었으며, 입원 의료비나 다양한 소득수준을 반영하지 못했다는 점에서 한계가 있다. 향후 연구에서는 총 의료비용과 다양한 저소득층 정의를 반영한 분석이 필요하다.
Abstract
This study analyzed the impact of the 2018 change in fixed copayment system for elderly outpatient services on elderly medical utilization, focusing on income levels. Using data from the National Health Insurance Service (NHIS) Elderly Cohort Database for the years 2016 to 2018, a multiple regression analysis with interaction terms was conducted on 461,225 individuals aged 65 and older. Following the policy reform, overall outpatient visits and insurer’s expenditures increased, while out-of-pocket payments decreased, indicating an improvement in healthcare accessibility among older adults. However, the effects of the reform differed by income level. Notably, among the low-income group, outpatient utilization in the “15,001–20,000 KRW” cost-sharing bracket—where the coinsurance rate was significantly reduced—actually decreased. This suggests that lowering the coinsurance rate alone may not be sufficient to encourage greater outpatient use, and that factors such as access to information about the policy changes may also influence utilization. However, this study is limited to outpatient care at clinics and does not account for inpatient medical expenses or various definitions of income level. Future research should incorporate total healthcare expenditures and consider multiple criteria for defining low-income groups.
