서울 지역 개원의의 대장암 조기검진 권고 실태

연구배경: 우리나라에서 대장암은 식생활의 서구화와 인구 고령화로 계속 증가 추세이다. 정기적인 조기검진으로 발생률과 사망률을 낮출 수 있고 특히 의사의 조기검진 권유가 중요함에도 불구하고 대장암 조기검진에 대한 의사들의 지식, 태도, 실행 여부에 관한 연구가 아직까지 국내에는 없는 실정이다. 본 연구는 서울 지역의 대장암 조기검진 현황을 파악하기 위해 대장암 조기검진 권고안에 대한 인식과 시행 여부, 조기검진 시행에 영향을 주는 외적 요인 등을 의료제공자인 의사의 입장에서 조사하였다. 방법: 서울 지역 내과, 외과, 가정의학과 개원의 760명에게 설문을 발송하였고 자료 수집은 2004년 3월 2일부터 4월 30일까지 시행하였다. 응답자 106명(응답률 14%)을 최종 분석 대상자로 정하여 분석하였다. 결과: 대장암 조기검진 시행률은 47.2%였으며 이는 대장암 조기검진 권고안에 대한 인지도(64.8%)나 동의율(93.2%)에 비해 낮은 수준이다. 장비 보유율은 대변잠혈반응검사(22.6%), S상결장경검사(16.0%), 대장내시경검사(16.0%), 대장 조영술검사(10.4%) 순이었으나 대장내시경검사(54.7%)를 가장 선호하였고, 그 중에 43%만 권고안에서 제시하는 검진 간격과 일치하였다. 많은 의사들이 대장암 조기검진 시행의 주요 장애요인으로 환자관련요인을 환경관련요인보다 중요하게 생각하였다. 결론: 대장암 조기검진의 필요성을 알지만 제대로 시행하지 못하는 데에는 여러 가지 의사, 환자 및 환경관련요인이 있었다. 이러한 장애 요인들을 충분히 고려한 권고안을 개발하여 지속적으로 평가하고 개선시킬 필요가 있다.
