지역 내 병․의원의 공급 부족이 노인 미충족 의료에 미치는 영향

초록

본 연구는 노인 미충족 의료에 영향을 미치는 지역의 의료 시설 분포 특성을 탐색하는 데 주된 목적을 두고 있다. 실증분석을 위하여 질병관리본부에서 구축하고 있는 지역사회건강조사를 사용하여 1수준 변수를, 전국 250개 시군구 단위의 병원, 의원을 2수준 지역 특성 변수로 고려하였으며, 분석방법으로 종속변수가 이항 형태인 임의절편모형을 적용하였다. 본 연구의 주요 분석결과는 병원, 의원과 같은 지역의 의료자원 분포가 노인 미충족 의료 확률에 통계적으로 유의한 영향을 미치고 있음을 보여주고 있다. 아울러 지역의 노인인구 비율이 높을수록 병. 의원 분포가 낮은 특성을 보이는 것은 인구 고령화에 따른 노인 인구의 미충족 의료 심화 개연성에 대한 정책적 대응의 필요성을 시사하고 있다.

Abstract

The purpose of this study is to explore whether the unmet medical care of the elderly is influenced by local characteristics such as the spatial distribution characteristics of the medical resources where they reside, in addition to the individual level characteristics such as demographic characteristics and household characteristics. For the purpose of empirical analysis, variables of 1 level were used in the community health survey established by the CDC, 250 hospitals and clinics were used as medical resources, respectively. The main results of this study show the significant influence of medical resource characteristics such as hospitals and clinics besides demographic characteristics and household characteristics on the probability of unmet medical care for the elderly. The proportion of elderly population and the negative correlation between health care resources suggests the need to improve the disparity of medical resources in terms of coping with the unmet medical needs of the elderly population, which is associated with population aging.
