한국 복지국가의 기원: 한국의 의료보험제도의 기원과 변화

초록

복지국가 또는 복지체제 연구는 한국에서는 저성장의 분야에 속하지만 정치학의 전통적 영역이다. 이 분야는 최근 서구에서 다시 크게 부활하고 사회과학 발전의 견인차 역할을 하고 있다. 에스핑-앤더슨은 물론이고 카메론, 윌렌스키, 스티븐스, 그리고 아주 최근의 피어슨에 이르는 정치학자들이 복지체제분야에서 중대한 기여를 하고 있다. 특히 피어슨 등 정치학의 최근 연구를 중심으로 하여 복지문제의 르네상스가 시작되었다. 그러나 복지국가에 관한 한, 한국의 시간대는 세계 시간대와 커다란 시간 격차가 있다. 한국의 정치학자들은 아직 복지에 대해 큰 관심을 보이지 않고 있다. 이 글의 문제의식은 복지국가문제를 한국정치학의 주요 영역에 넣으려는 시도의 일환이다. 이 글의 목적은 한국복지체제를 그 연원으로부터 검토하는 것이다. 연원에 대한 연구는 에스핑-앤더슨 이후의 연구동향을 고려할 때 대단히 중요하다. 에스핑-앤더슨의 역저 복지자본주의의 세 가지의 제목 자체가 보여주는 것처럼, 복지체제의 발전이나 변화는 직선적이기 보다 각 사회의 역사적 현실을 반영하여 다양하게 발전한다. 마찬가지로 한국에서의 복지국가의 생성, 기원 및 이후의 발전을 조사하기 위해서는 각 프로그램의 역사적 기원에 대한 조명이 필수적이다. 글의 첫째 부분은 복지국가 이론에 대한 최근의 활발한 문헌을 검토하는 동시에 비교연구를 위해 비스마르크의 사회정책을 소개할 것이다. 복지국가 연구는 최근 이론이나 경험연구의 모든 측면에서 급성장 분야를 형성하고 있다. 냉전 종식과 더불어 국제정치 연구가 쇠퇴하는 반면, 국제정치경제와 비교정치가 통합되는 추세 속에서 통합의 접점을 대표적으로 보여주는 복지체제 연구는 가장 활성화되고 있다. 둘째 부분은 국민보험제도와 더불어 복지국가의 근간을 형성하는 의료보장제도의 역사적 전개를 역사적 관점에서 조망하는 것이다. 의료복지제도의 주요 계기가 모두 군사 정부하에서 있었던 점에 주목하여 그 이유를 밝히는 데 힘을 쏟을 것이다. 세 번째는 최근의 의료개혁문제를 의료보장제도의 역사적 탄생과 관련하여 설명한다.

Abstract

The Origins and Development of Medical Insurance in Korea Kang, Myong-Sei This paper aims to explain the origins and development of medical insurance in Korea. Medical insurance determined the essential nature of the welfare state with accident, disability and retirement insurance. Medical insurance was legislated during the authoritarian regime of Park Chung Hee. Medical benefits during Chun’s authoritarian government were extended to cover the entire population in 1988. I argue that medical insurance was made by bureaucratic initiatives. Authoritarian regime had two purposes in initiating medical insurance. As Bismarck found the welfare state in Imperial Germany to buy the social peace, motive behind legislation of medical insurance was political. The Park government reformed medical insurance in 1976, recognizing that North Korea had provided national insurance. It was a part of regime competition with North Korea. However, universalism was weak because medical insurance benefits have been contributory from the beginning. Difficulties in recent reforms of medical insurance, highly controversial, reflect this institutional setup.
