열공망막박리에서 유리체절제술과 수정체유화술 동시 수술과순차적 수술의 비교

목적: 수정체안 열공망막박리 환자에서 유리체절제술 및 수정체유화술을 동시에 시행한 경우와 유리체절제술 단독 시행 후 수정체유화술을 시행한 경우의 결과를 비교해 보았다. 대상과 방법: 수정체안 열공망막박리로 진단받은 환자를 유리체절제술 단독 시행 후 수정체유화술을 시행한 경우(순차적수술군)와유리체절제술과 수정체유화술을 동시에 시행한 경우(동시수술군)로 나누어 최대교정시력, 수술성공률, 수술 후 합병증을 비교하였다. 결과: 순차적수술군은 23안, 평균 나이 54.17세, 동시수술군은 16안 56.69세로 동시수술군의 평균 나이가 많았다(p=0.031). 최대교정시력(logMAR)은 순차적수술군과 동시수술군 모두에서 의미 있는 시력 상승을 나타냈으나 두 군 간 통계학적으로 유의한 차이는 없었다(p=0.592). 순차적수술군과 동시수술군 모두 일차수술의 해부학적 성공률은 100%였고, 순차적수술군 중 3안에서 백내장 수술 후새로운 열공에 의해 망막 재박리가 발생하였다. 결론: 순차적수술군에서 새로운 열공망막박리의 빈도가 높았으나 두 군 간의 연령이 다르고 연구대상자 수가 적은 후향적 관찰연구로열공망막박리 환자에서 새로운 열공망막박리의 위험을 줄이기 위해 단독 유리체절제술보다 유리체절제술과 백내장 동시수술이 타당하다는 결론에는 한계가 있으며 더 많은 증례로 전향적인 연구가 필요할 것으로 생각한다.
Abstract
Purpose: To compare the outcomes of phacovitrectomy and phacoemulsification after vitrectomy for treatment of rhegmatogenousretinal detachment (RRD). Methods: We performed a retrospective comparative analysis of 39 consecutive eyes with phakic primary RRD followed up formore than 6 months. The patients were divided into phacoemulsifcation after vitrectomy and phacovitrectomy groups. The mainoutcome measures were the best corrected visual acuity (BCVA), anatomical success rate and postoperative complications. Results: The mean age was 54.17 years in the phacoemulsifcation after vitrectomy group (n = 23) and 56.69 years in the phacovitrectomygroup (n = 16; p = 0.031). The log MAR BCVA improved in both groups with no statistically significant difference betweenthe 2 groups (p = 0.592). The anatomical success rate after initial surgical intervention was 100% in both groups. Retinaldetachment recurred in 3 eyes in the phacoemulsifcation after vitrectomy group; caused by new retinal tear. Conclusions: The new RRD rate in phacoemulsification after vitrectomy group was higher than in the phacovitrectomy group. Due to the retrospective and limited data in this study, whether simultaneous combined cataract surgery with retinal detachmentsurgery should be recommended to reduce RRD risk is inconclusive and further larger, prospectively designed studies are necessaryto confirm the present findings.
