산전 부신피질 호르몬 요법이 태아 성장에 미치는 영향

초록

=목적: 산전 부신피질 호르몬 요법이 태아 성장에 미치는 영향을 분석하고, 부신피질 호르몬의 투여 횟수에 따른 태아 성장의 차이를 비교하고자 하였다.연구 방법: 임신 24주에서 34주 사이에 조산의 위험으로 입원한 단태 임산부 797명을 대상으로 부신피질 호르몬 투여 유무와 횟수에 따라 비사용군과 1회 사용군 및 2회 이상의 다회 사용군으로 분류하여 후향적 고찰을 시행하였다. 연구 대상 임산부의 분만까지의 경과를 관찰하여 신생아의 출생 체중, 두위, 복위 및 흉위와 재태 연령에 따른 과소 출생 체중 (small-for-gestational age) 여부를 조사하였다. 결과: 비사용군, 1회 사용군, 그리고 다회 사용군에 각각 295명, 409명, 93명이 포함되었다. 신생아의 출생 체중, 두위, 복위, 흉위 및 과소 출생 체중의 발생 빈도는 각 군간 유의한 차이를 보이지 않았다. 재태 연령에 따른 출생 체중, 두위, 복위 및 흉위의 곡선을 구하여 각 군의 곡선을 출생 시 재태 연령을 보정한 후 비교한 결과 신생아의 출생 체중, 두위, 복위 및 흉위는 각 군간에 유의한 차이가 없었다. 재태 연령에 따른 과소 출생 체중의 발생 빈도 역시 출생 시 재태 연령을 보정한 후에도 각 군간에 차이가 없었다.
