지역기반 보건⋅복지 연계 가능성에 대한 탐색연구 - 노인의 생애 말기 돌봄서비스를 중심으로

지역사회는 다양한 삶의 양식과 더불어 다양한 욕구가 존재하고 있는 공간이다. 복지환경 및 사회환경의 변화로 인해 다양한 욕구 중, 삶의 마지막인 생애말기(End-of-life)에 대한 욕구도 존재하며, 이 시기와 밀접하게 관련있는 노인의 경우복지와 보건의 욕구가 복합적으로 나타나고 있다. 이에 본 연구에서는 생애말기돌봄의 사업을 실행하고 있는 공공형 부산광역시 호스피스 사업에 참여하고 있는 전문가, 보건기관실무자, 복지기관실무자 총 36명을 대상으로 2015년 2월부터 5월까지 초점집단인터뷰(FGI)를 실행하여 지역사회의 보건과 복지의 연계가능성을 탐색하고자 하였다. 조사결과 첫째, 지역사회에서 보건복지연계가 필요하고 둘째, 협업과 연계가 되기 위해서는 협업을 위한 합의가 필요하였으며, 셋째, 연계의 핵심은관계였다. 넷째, 각자의 영역에서 전문성이 있어야 연계가 가능하다는 점을 알 수 있었다. 본 연구를 결과를 통해 보건과 복지의 연계 가능성은 있고, 보건과 복지의 영역에서 전문성을 기반으로 하는 접근을 할 경우, 대상자의 복합적인 문제를 해결할 수 있는 좋은 복지·보건모델이 제시될 것으로 기대된다.
Abstract
The local community that has close ties with our lives is a place where various life styles along with desires coexist. Due to he changes in community welfares and social environment created different desires, including the desire for quality of life at the end-of-life. There has been a complex appearance in desires for welfares and healthcare during the end-of-life quality of living for elderly citizens within local community. For that reason, this research seeks to explore the possibility of connection for health and welfare within the local community by conducting Focus Group Interview (FGI) with total of 36 experts, healthcare staffs and welfare staffs from February to May 2015. The research result is like the following. First, there's a huge need for health and welfare connection within local community. There has been an existing complex desire for health and welfare within the local community along with natural associations, and there's also been overlapping problems about the recipient of health and welfare related care. Second, In order to be associated with cooperative work, there needs to be a partnership agreement. Continuous mutual monitoring, using public systems such as official documents and integrated systems, and specification of main agent are important factors and both parties should compliment one another based on clearly defined roles. Third, main focus of such association is relationship. There is a need for understanding each other's studies as well as a need for mutual understanding through meaningful conferences and case meetings. Forth, it was clear that in order for such liason to take place, each party must have expertise in their domain. Discussion and implication based on such research result are like the following. Since the local community is a dynamic and organic entity, the main source of successful social welfare implementation is to find areas within the community that falls short on performance and revitalize it. There's a high expectation that this research will contribute to developing a successful social and health care services model which can resolve complex issues for its recipients by implementing professionalism within social and health care industry.
