직장의 출혈성 Dieulafoy 병변의 치료를 위한 내시경 치료 방법들, 가령 전기응고, 클립치료, 에피네프린과 경화제주입은 외과적 치료의 필요성을 감소시켰다. 최근에 내시경적 밴드결찰술은 직장질환의 치료를 위해 좋은 결과를 보여 주었다. 이러한 치료방법은 Dieulafoy 병변, Mallory-Weiss 병변, 혈관기형과 같은 비섬유화조직내에의 출혈치료를 위해 적절하다. 이에 저자들은 직장 Dieulafoy 병변으로 인한 직장출혈 환자에서 내시경적 밴드결찰술로 성공적인 치료를 한 예를 보고하고자 한다.
Abstract
The variety of endoscopic therapy for the treatment of bleeding Dieulafoy's lesion of the rectum, such as electrocoagulation, clipping, epinephrine and sclerosant injection, reduced the necessity of surgical intervention. Recently, endoscopic band ligation was introduced with favorable results for the hemostasis of rectal lesions. This type of therapy is appropriate for areas of bleeding within otherwise non-fibrotic tissue, such as Dieulafoy's lesion, Mallory- Weiss tear and vascular malformations. We report a case of successful management with endoscopic band ligation in a patient with rectal bleeding from Dieulafoy's lesion. (Korean J Gastrointest Endosc 2002;24:235-238)
