점액 과분비성 다발성 담도 유두종 1예 -국내 문헌 고찰을 중심으로-

점액 과분비성 담도 종양은 드문 질환으로 임상적, 방사선학적, 병리학적 특징들이 잘 알려져 있지 않다. 저자 등은 최근 근치적 절제술을 시행한 점액 과분비성 다발성 담도 유두종에서 선종으로부터 선암으로 이행되었다고 추정되는 1예를 경험하였기에 이를 점액 과분비성 담도 종양에 관한 11개의 국내 문헌 고찰과 함께 보고하고자 한다. 65세 여자가 발열과 오한을 동반한 심와부 및 우상복부 통증을 주소로 내원하였다. 복부 전산화 단층 촬영에서 심한 좌측 간내 담관의 확장과 담도내 음영 결손이 관찰되었다. 내시경적 역행성 담췌관조영술에서 유두 개구부를 통해 다량의 점액이 배출되었으며 총담관이 늘어나 있으면서 부정형의 음영 결손이 관찰되었다. 경피경간 담도경 소견에서 다량의 점액과 산호 모양의 다발성 유두상 돌기들이 좌측 간내 담관에서 관찰되었다. 환자는 좌측 간 절제술을 시행 받았고 병리학적으로 유두상 선종의 배경에서 유두상 선암으로 진행된 소견이었다.
Abstract
The mucin-hypersecreting bile duct tumor is rare and its clinical, radiologic, and pathologic features are not well known. We report the case of mucin-hypersecreting biliary papillomatosis with malignant transformation including review of 11 korean literatures. A 65-year-old female was admitted to our hospital with fever, chills, and right upper quadrant pain. A CT scan showed marked dilatation of left intrahepatic duct with intraductal filling defects. During endoscopic retrograde cholangiograpy, mucin from the ampulla was observed and dilated common bile duct with multiple amorphous filling defects was noted. Cholangioscopy revealed muliple coral-like mucosal papillary projections with large amount of mucin in the left intrahepatic ducts. The patient underwent left lobectomy, and the biopsy revealed intraductal papillary adenocarcinoma in the background of papillary adenoma. (Korean J Gastrointest Endosc 2003;26:167-171)
