초록

아니사키스증은 아니사키스 유충이 기생하는 해산어류나 두족류를 생식하였을 때 발생하며 이들의 유충이 위나 소장벽에 침입하여 위장관의 급만성 병변과 심한 복통, 오심, 구토 등의 증상을 일으킨다. 대부분 해산물을 생식한 후 1～2일에 위내에서 주로 발견된다. 지금까지 상부위장관 내시경 검사를 통해 위벽 내의 아니사키스증을 진단한 예는 있으나 소장이나 대장에서의 발견은 드물었다. 우리 병원은 서해안 지역에 위치하고 있으며 지역적 특성상 회를 많이 섭취하는 주민들의 식습관과 아울러 최근 비약적인 발전을 이룬 대장내시경의 보급과 기술력의 증가로 대장내시경을 통한 대장에서의 여러 질환들을 경험하였다. 그중 저자들은 생선회를 먹고 다양한 증상을 주소로 내원한 각각 다른 세 명의 환자들에서 대장내시경검사를 시행하여 우연히 맹장, 상행결장, 횡행결장, S상결장 등 대장의 여러 부위에서 아니사키스를 발견하여 내시경적으로 제거하였기에 문헌고찰과 함께 보고하는 바이다.

Abstract

Anisakiasis occurs when humans ingest raw or inadequately cooked saltwater fish or squid that is infested with anisakis larvae. The larvae invade the gastrointestinal mucosa and cause abdominal pain, nausea, vomiting and various gastrointestinal lesions. Anisakiasis of the stomach has been reported occasionally but involvement of the small bowel or colon is extremely rare. We report three cases of anisakiasis in the colon with a brief review of the relevant literature. (Korean J Gastrointest Endosc 2006;33:239-243)
