좌측 대장직장암 폐색에서 스텐트 삽입 후근치 수술의 유용성 및 비용-효과 분석

Usefulness and Cost-effectiveness of Colorectal Stent Followed by Curative Resection for Left-sided Malignant Colorectal Obstruction

초록

목적: 좌측 대장직장암 폐색의 근치 치료에 있어 스텐트 삽입 후 감압을 유도하고 수술하는 술식이 보편화되고 있다. 본 연구에서는 폐색을 동반한 좌측 대장직장암에서 스텐트 삽입 후 수술이 응급 수술에 비해 임상 및 비용-효과 면에서 우월한 지 알아보고자 하였다. 대상 및 방법: 2005년 1월부터 2007년 6월까지 좌측 대장직장암 폐색으로 진단되어 근치 치료를 받은 환자 중 스텐트 삽입 후 수술한 24명(스텐트 군)과 응급 수술을 시행한 22명(응급 수술 군)의 임상 경과 및 진료 비용을 비교 분석하였다. 결과: 스텐트 군과 응급 수술 군의 연령(60.6±3.1세 vs. 62.1±3.2세, p=0.74) 및 남녀 비(12：12 vs. 15：7, p=0.25)는 차이가 없었다. 수술 후 병리학적 병기는 스텐트 군에서 2기 및 3기가 각각 11명 및 13명으로 응급 수술 군의 11명 및 11명과 차이가 없었다(p=1.00). 스텐트 군은 모든 환자가 1회 수술로 근치 절제가 가능하였으나 응급 수술 군에서는 6명(27.3%)이 2회 이상의 수술을 받았다(p＜0.01). 스텐트 군의 재원 기간은 22.0±0.8일이었고 응급 수술 군은 26.3±2.4일이었다(p=0.09). 수술 후 중환자실 치료를 필요로 한 경우는 스텐트 군이 1명(4.2%)으로 응급 수술 군의 7명(31.8%)보다 적었다(p=0.02). 스텐트 군에서는 수술 소견상 스텐트 근위부에 천공이 발생한 1명을 제외한 수술 후 합병증은 없었으나, 응급 수술 군에서는 창상 결손 3명, 신부전 1명, 장문합부 누출과 장피누공이 각각 1명에서 발생했다(p=0.04). 재원 기간 중 진료비는 스텐트 군이 평균 7,974,236원으로 응급 수술 군의 9,271,630원보다 약 14% 절감되었다(p=0.06). 결론: 좌측 대장암 폐색에서 근치 수술 전 스텐트 삽입은 응급 수술에 비해 비용 절감 경향을 보이면서 수술 횟수 및 합병증 발생, 중환자실 치료 등에서 우월하여 비용-효과 면에서 우수하다.
