Background : Heat stroke, a rare life-threatening medical emergency,is known to cause CNS dysfunction such as cerebral edema, cerebral hemorrhage and cerebellar dysfunction. Case report : A 75-year-old woman who had no other health problem was brought to the emergency room after being found on a road in a comatose mental state. On admission, her systolic blood pressure was 40mmHg and body temperature was 41.3 ℃. Her laboratory findings were consistent with multiorgan failure. A large bore intravenous line was initiated and normal saline solution was administered. Ice bags were placed in the axillae and groin bilaterally in an attempt to lower her core body temperature. After treatment, her mentality improved to drowsy state but quadriparesis remained. Brain MRI showed multiple cerebral infarctions involving the bilateral basal ganglia, right cerebellum, hippocampus and bilateral periventricular white matter. Conclusions : We suggest that cerebral infarctions may be caused by global cerebral ischemia associated with heat stroke. Korean Journal of Stroke 2001;3(2): 168~170
