Neuroimaging features in hypertensive encephalopathy include diffuse or focal hyperintensities on T2-weighted images, predominantly in supratentorial white matter, especially in occipital lobes. We report an unsual case of hypertensive encephalopathy who showed brain stem hyperintense signal in T2 weighted MRI combined with acute ischemic supratentorial stroke. The lesions improved dramatically after stabilization of blood pressure, suggesting that edema could be the main cause of the MR imaging. Korean Journal of Stroke 2001;3(1): 115~117
