Background: The pathogenesis of cerebral infarction in malignancy is complex and not yet completely understood. A hyper-coagulable state, non bacterial thrombotic endocarditis and thrombotic arterial microangiopathy may be important causes of cerebral infarction in malignancy. The purpose of this study is to describe diffusion-weighted imaging (DWI) patterns of cere-bral infarction in patients with malignancy. Methods: We analyzed 28 patients with cerebral infarction and malignancy. We reviewed medical records and retrospectively analyzed the DWI scans in all patients. The presence, size, number, and loca-tion of all hyperintense lesions in the DWI were analyzed. Results: We classified the DWI lesions into 4 patterns; 1) single middle to large sized lesion (3 cases, 10.7%), 2) single small sized lesion (5 cases, 17.9%), 3) multiple lesions in single ar-terial territory (10 cases, 35.7%), 4) multiple disseminated lesions (10 cases, 35.7%). Single lesions were present on 8 scans (28.6%) and multiple DWI lesions were present on 20 scans (71.4%). Conclusion: Cerebral infarction associated with malig-nancy usually have multiple and variable sized lesions. DWI patterns could provide some clues to detect malignancies when etiology of cerebral infarction is ambiguous. (Korean J Stroke 2008;10:36-40)
