빈혈이 있는 위궤양 환자에서 에피네프린 주입 후 발생한 위괴사 1예

에피네프린 점막하 주입은 비교적 안전하고 시술 방법이간단하며 천공이나 주위 조직 괴사가 잘 일어나지 않으므로널리 이용되는 위장관 지혈술 방법 중 하나이다. 위는 복강동맥으로부터 풍부한 혈액을 공급 받으며 위벽 내에 측부혈관이 발달해 있어 허혈성 위괴사는 드물다. 현재 전 세계적으로 보고된 문헌을 살펴보면 에피네프린 점막하 주입 후발생한 허혈성 위괴사가 8예로 매우 드물고 모두 고령, 당뇨병, 고혈압, 협심증 등의 동맥경화의 위험인자가 있는 환자들이었다. 저자들은 빈혈 이외에 특별한 질환이 없는 젊은환자에서 에피네프린 점막하 주사 후 발생한 허혈성 위괴사를 경험하였기에 문헌고찰과 함께 보고한다.
Abstract
Endoscopic epinephrine injection is a safe and easy treatment for non-variceal gastrointestinal bleeding. It has low complicationrates and is used widely. Ischemic gastric necrosis occurs rarely because of the rich vascular supply of the stomach and the vascularreserve of the intramural anastomosis. Endoscopic injection therapy, smoking, hypertension, and atherosclerosis are risk factors forgastric ischemia. There have been a few case reports of gastric ischemia after endoscopic injection therapy. We report a case of gastricischemia after submucosal epinephrine injection in a 29-year-old woman with anemia.
