세 포진 이상(ASC-US, LSIL, ASC-H, HSIL 등)에서 HPV 양성률이 높았 고, ASC-US와 고위험 HPV 동시 양성률은 43.8%였다 (P < 0.05).
Abstract
Background: Early screening for cervical cancer is crucial for identifying precancerous stages. Human papillomavirus (HPV), particularly geno- types 16 and 18, plays a key role in its development. This study investigated high-risk HPV genotypes within the Korea Association of Health Pro- motion (KAHP)’s cohort and performed a comparative analysis of cervical cytology and HPV genotyping. Methods: This study included 16,758 cases from 16,097 women who underwent both cervical cytology and high-risk HPV genotyping. Cervical specimens were classified according to the 2014 Bethesda System. HPV genotyping was conducted using the Anyplex™ II HPV HR Detection Kit. Chi-square tests assessed associations between variables. Results: High-risk HPV was detected in 25.5% of cases. The most prevalent genotype was HPV 52, followed by 58 and 51. Genotype-specific prevalence varied significantly across age groups and cytologic categories. HPV 58 showed the highest prevalence in women aged 20–29 years and ≥ 70 years. Cytologic abnormalities, including LSIL (low-grade squamous intraepithelial lesion), ASC-H (atypical squamous cells, cannot ex- clude HSIL), and HSIL (high-grade squamous intraepithelial lesion), were strongly associated with HPV positivity. Co-positivity of ASC-US (atypical squamous cells of undetermined significance) and high-risk HPV was 43.8%. Chi-square analysis confirmed significant differences in HPV geno- type distributions across age and cytology (P < 0.05). Conclusions: These findings support the clinical value of integrating HPV genotyping with cytology in national screening programs. Genotype- and age-specific data may inform targeted vaccination and triage strategies for cervical cancer prevention.
