알레르기비염은 흔한 만성 질환으로, 치명적 질환은 아니지만 삶의 질을 떨어뜨리고 사회경제적 부담을 가중시켜 환자의 삶을 힘들게 할 수 있어, 적절한 관리가 필요한 질환이다. 비염에는 알레르기비염뿐 아니라 비알레르기비염(nonallergic rhinitis)도 있다. 알레르기비염은 43%, 비알레르기비염은 23%, 이들의 혼합형(mixed)은 34%로 보고되어 있다.1 치료는 회피요법, 약물요법, 면역요법, 수술요법 중에서 가능한 방법으로 시행한다. 최근 Allergic Rhinitis and its Impact on Asthma 가이드라인에서는 비염의 중증도에 따라 비염약물을 달리 사용하는 것을 제안하고 있다. 한편, 최근 천식에서 약물요법은 중증도 개념에서 조절 개념으로 바뀌고 있다. 즉 천식의 조절 정도를 보고 조절이 안되면 약물 단계를 올리고 조절이 잘되면 단계를 내리는 것으로, 단계적 약물요법을 시행하는 것이다.2 비염도 천식과 마찬가지로 조절 수준을 평가하여 이러한 방법으로 약물 치료가 제안될 가능성이 높다.3,4 그러나 천식이나 비염에서 조절 수준을 평가하는 방법이 실제 임상에서 쉬운 것만은 아니다. 제한된 짧은 시간에 많은 환자를 진료해야 하는 의료 시스템을 가지고 있는 국내 경우에는 더욱 그렇다. 만약 환자가 직접 설문지를 작성하고 이 점수를 가지고 의사가 판단한다면 편리할 수도 있다. 천식에서는 천식조절검사(asthma control test, ACT)5를 포함하여 몇 가지가 이미 알려져 있고, 임상에서 사용하기도 한다. ACT는 5문항으로 되어 있고, 각 문항당 1–5점으로 되어 있으며, 점수가 높을수록 조절이 잘되는 것으로 평가한다. 미국 천식 치료 지침에서 천식 조절 수준을 평가하는 방법 중 하나로 ACT 사용을 제안하였으며,2 ACT 한국어판도 나와 있어 실제 임상에서 사용하고 있다.
