체외수정 및 배아이식의 도입이래 자궁외 임신의 빈도가 증가하고 있다. 위험 인자로는 불임의 원인이 되는 난관 인자, 자궁근종 절제술의 기왕력, 이식되는 배아의 수, 배아 이식시의 기술적 측면, 배아 이식시 배양액의 양, 배아 이식 후 역방향으로의 자궁 수축 등을 들 수 있다. 체외수정 및 배아이식이후 추적 검사시 복합성 임신의 가능성을 고려하여, β-hCG와 함께 질식 초음파로 자궁강 내의 태낭유무를 관찰하면서 동시에 양측 부속기를 면밀하게 검사함으로써 자궁외 임신을 조기 발견하여 혈역동학적으로 안전한 상태에서 치료할 수 있다. 저자들은 체외수정 및 배아이식이후 좌측 난관과 우측 난소에 동시에 생긴 자궁외 임신 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
Abstract
The incidence of ectopic pregnancy is increased since IVF-ET has started. The risk factors for ectopic pregnancy after assisted reproductive techniques are tubal abnormality, previous myomectomy, the number of transfered embryo, the technical aspects of embryo transfer, the amount of transfered medium and uterine reverse contraction after embryo transfer. Because of the possibility of combined pregnancy after IVF-ET, bilateral adnexal area should be examined by transvaginal ultrasonography at searching intrauterine gestational sac and also serial β-hCG levels should be checked. We report a case of simultaneous left tubal and right ovarian pregnancies after IVF-ET with a brief review of literature.
