전치 혈관은 약 2,000-5,000 분만 중 1건 정도로 발생하는 드문 질환으로 태아측 혈관이 자궁내구와 태아 선진부 사이에 위치하게 되어 분만진행 중 쉽게 파열될 수 있으며, 그로 인하여 태아 사망까지 이르는 치명적인 결과를 초래할 수 있다. 그러나 매우 낮은 발생률로 인하여 주의를 기울이지 않는다면 올바른 진단이 이루어지기 어려우며, 분만 후에는 진단이 더욱 어려운 질환이다. 따라서 분만 전 올바른 진단이 이루어지고 적절한 처치를 한다면 그 주산기 예후는 매우 향상될 것이다. 저자들은 임신 29주에 전치태반 의심 하에 본원 전원된 산모에서 임신 32주경에 추적 초음파 검사 및 색 도플러 초음파 상에서 전치 혈관을 진단, 임신 35주에 질 출혈로 제왕절개술을 시행하여 정상 신생아를 분만하였고 이후 태반 병리 소견상 제대 난막부착 소견을 확인한 전치혈관 산모 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
Abstract
Vasa previa is one of the most unusual and tragic accidents to fetus in obstetrics. This condition is not diagnosed easily because it is rare and unexpected. To improve the perinatal outcome, early detection by ultrasonography is very important during pregnancy. We present a case of vasa previa with velamentous insertion of cord with a brief review of the literatures concerned.
