유두양 장액성 자궁내막선암은 다른 자궁내막암들 보다, 드물지만 림프혈관내 침습 경향과 전신적 파종 경향이 매우 높다는 특징을 가지고 있으며, 난소의 유두양 장액성 난소암에서와 유사하게 복막으로의 전이 또한 특징적으로 나타난다. 그리고 재발률과 사망률이 높은 악성 질환으로 평가받고 있는 상황이다. 이러한 이유들로 인하여, 이 종양에 연관된 유전자들에 대한 고찰과 함께 치료법에 대한 연구들이 현재 진행 중에 있다. 많은 연구들에서, 시험 개복술을 시행하여, 전자궁 절제술, 양측 난소 난관 절제술, 대망 절제술, 골반내 림프절 절제술, 방대동맥 절제술, 그리고, 복수의 세포학적 검사를 기본적으로 시행할 것을 권고하고 있다. 이와 더불어 수술 후 전신적 항암제 투여와 추가 방사선치료를 행하는 것이 도움이 된다고 밝히고 있다. 본원에서는 수술전 검사상 자궁 내막암으로 진단되어 수술 후 유두양 장액성 자궁내막선암으로 판명된 증례를 경험하였기에 분자 유전학적 연구와 치료법들에 대한 문헌들을 고찰하고 보고 하는 바이다.
Abstract
Uterine papillary serous carcinoma (UPSC) behave more aggressively than other endometrial carcinomas and have a propensity for intraabdominal spread, simulating the behavior of ovarian carcinoma. Because of high relapsing rate, and high mortality rate of UPSC, many gynecologist studied about its treatment regimen and recommended many treatment method. Many investigators recommended that patients with UPSC should undergo a staging laparotomy and they suggested the surgery should include at least total hysterectomy, bilateral salpingo-oophorectomy, omentectomy, pelvic lymphadenectomy, paraaortic lymphadenectomy, peritoneal washing and peritoneal cytology, By and large, adjuvant systemic Platinum based chemotherapy or, paclitaxel based chemotherapy and adjuvant whole abdominal irradiation or pelvic irradiation was prescribed. We experienced two cases of the UPSC stage Ⅲc and stage Ⅳ diagnosed after explolaparotomy. We present these cases and review the literatures about the optimal treatment regimen of UPSC.
