한국 분만의료기관의 산전 검사 시행 실태에 관한 조사 및 산전 검사 권고안 마련을 위한 제언

초록

=목적: 미국을 비롯해 여러 나라들이 자국 실정에 맞는 산전 검사 권고안을 마련해 놓고 임상에 도움을 주고 있다. 그러나 우리나라는 아직까지 현재 임상에서 시행되고 있는 산전 검사 실태조차 파악이 미흡한 실정이다. 이에 우리는 한국 분만 의료기관에서 시행하고 있는 산전검사의 시행실태에 대해 조사해 보고자 하였다.연구 방법: 2001년 국민 보험 공단에서 집계한 연간 100건 이상 분만이 이루어진 868개 분만의료기관 중, 주소가 확실한 848곳에 대한 횡단적 서술적 조사 연구를 통해 산전 검사 시행 실태를 파악하고, 문헌 조사를 함께 하였다. 결과: 산전 검사 종류에 따라 80% 이상의 분만의료기관에서 일률적으로 시행되는 항목, 10% 미만의 의료기관에서만 기본적으로 시행하고 있거나 고위험 임신 산모에게만 시행되는 항목, 산모의 요구나 주치의의 경험에 의해 시행되는 항목으로 다양한 형태로 산전 검사들이 시행되고 있었다. 고찰: 임상에서 시행되고 있는 산전 검사들이 선별검사로서의 가치가 정확히 평가되지 않은 실정이다. 선별검사란 관점에서 산전 검사 각 항목에 대한 임상 평가를 시행하고 이를 바탕으로 우리 실정에 맞는 체계적이고 효율적인 산전 검사 권고안을 마련하고자 하는 노력이 필요하다.
