난소 내배엽동 종양에 대한 임상병리학적 분석과 예후

=목적: 난소 내배엽동 종양의 임상병리학적 특징 및 예후에 대해 알아보고자 한다.연구 방법: 1991년 9월부터 2005년 2월까지 본원 산부인과에서 난소 내배엽동 종양으로 진단되어 치료받은 17명의 환자를 대상으로 의무기록을 통해 임상병리학적 특징, 치료 방법, 치료 결과를 후향적으로 조사하였다. 결과: 17명 환자들 연령의 중앙값은 22.0세 (range 7-43)였으며 병기별 분포는 9명 (52.9%)이 병기 I, 3명 (17.6%)이 병기 II, 4명 (23.5%)이 병기 III, 1명 (5.9%)이 병기 IV였다. 12명의 환자에서는 가임력을 보존하기 위하여 일측 자궁부속기 절제술 또는 일측 난소절제술을 시행하였고 5명의 환자에서는 전자궁적출술 및 양측자궁부속기 절제술을 시행하였다. 모든 환자들에게 수술 후 보조적인 복합항암화학요법을 시행하였으며 11명은 BEP (bleomycin, etoposide, cisplatin) 복합항암화학요법을, 6명은 환자에서는 VAC (vincristine, actinomycin D, cyclophosphamide) 복합항암화학요법을 시행하였다. 1명의 환자에서 재발 소견을 보였으며 3명의 환자가 사망하였다. 평균 생존기간은 153.6개월이었고 병기 I, II인 환자들에서 병기 III, IV인 환자에 비해 통계학적으로 양호한 예후를 보였으며, BEP 복합항암화학요법을 시행한 환자들에서 VAC 복합항암화학요법을 시행한 환자들에 비해 양호한 예후를 보였다 (p<0.05).
