비정상자궁출혈에서 식염수 주입 초음파 자궁조영술의 진단적 가치

=목적: 질식초음파 (transvaginal ultrasonography)는 현재 여성생식기 질환을 진단하는 일차적인 진단적 도구로 사용되고 있다. 이에 더하여 식염수 주입 초음파 자궁조영술 (transvaginal ultrasound guided saline infusion sonohysterography, SIS)은 자궁강내의 이상을 진단하는 또 다른 진단적 방법으로 알려져 있다. 비정상자궁출혈 원인을 진단함에 있어 단순 질식초음파와 비교하여 SIS의 효용성을 평가하고자 한다.연구 방법: 2002년 9월부터 2004년 12월까지 비정상자궁출혈의 증상이 있는 가임기여성과 폐경여성 88명을 대상으로 하여 1차적으로 질식초음파를 시행한 후 SIS를 추가 시행하였다. 최종 진단은 자궁경하에서 조직을 얻어 확인하였고 그 결과에 따라 질식초음파와 SIS의 결과를 비교 분석하였다. 결과: 비정상 자궁출혈이 있는 88명의 여성을 대상으로 질식초음파, SIS, 자궁경하 자궁내막조직검사를 시행하였다. 질식초음파에서는 88명 여성 중 자궁내 병소가 보인 경우는 자궁내막용종이 15명, 점막하 자궁근종이 14명, 불규칙자궁내막이 28명이었고, 자궁내막층에 특이소견 보이지 않은 경우가 31명이었다. SIS에서는 88명의 여성 중 자궁내막용종이 17명, 점막하 자궁근종이 17명, 불규칙자궁내막이 23명이었고 자궁내막층에 특이 소견 보이지 않았던 경우가 28명이었고 세명은 시술에 실패하였다. 자궁경하 자궁내막조직검사에 의한 최종조직검사결과 자궁근종 17명, 자궁내막용종 19명, 자궁내막증식증 11명, 만성자궁내막염 2명, 증식기나 분비기 자궁내막 등 특이한 병변이 없었던 경우가 38명이였고, 그 외 위축성 자궁내막염, 탈락막반응이 각각 1명이었다. 최종 조직검사결과에 근거하여 질식초음파의 민감도는 72.9%, 특이도는 45%, 양성예측도는 61.4%, 음성예측도는 58.1%였고, SIS의 민감도는 87.8%, 특이도는 61.1%, 양성예측도는 75.4%, 음성예측도는 78.6% 였다.
