폐경 후 자궁 출혈에 대한 자궁경의 효과

The diagnostic role of hysteroscopyin postmenopausal bleeding

초록

=목적: 질식 초음파에 의한 자궁내막 두께와 자궁내시경의 진단적 정확도를 비교하여 폐경 후 비정상적인 자궁출혈이 있는 여성에서 자궁강 내 이상소견과 자궁내막암의 적절한 진단 방법을 평가하는데 있다. 연구 방법: 후향적으로 병력검사에서 출혈을 일으킬만한 약을 복용하지 않은 여성에서 폐경 후 자궁출혈을 호소한 86명과, 1년 이상 호르몬 치료를 하였으나 최근 10일 이상 비정상출혈을 한 19명, 총 105명을 대상으로 질식초음파, 자궁경 검사, 조직 검사를 시행하였다. 자궁내막의 두께를 측정하기 위한 초음파는 질식초음파 (Aloka, Japan, 11Hz) 를 이용하였으며 동일인에 의해서 평가되었다.결과: 질식 초음파상에서 자궁내막 두께가 4 mm 미만인 환자는 42명 (40%), 4 mm 이상 8 mm 미만인 경우는 49명 (46.7%), 8 mm 이상은 14명 (13.3%)이었다. 자궁경 검사에서 자궁내막 두께가 4 mm 미만인 경우 42명 중 30명 (71.4%)에서 자궁내막 위축 소견을 보였으며, 1명 (2.4%)에서 자궁내막암이 진단되었다. 자궁내막 두께가 4 mm 이상인 63명 중 폴립과 근종에 의해서 출혈한 경우는 31명 (49.2 %)이었으며, 자궁내막암은 3명 (4.8 %)이었다. 폐경 후 호르몬 치료를 한 19명의 여성 중 1예에서 자궁내막암이 발견되었다. 질식 초음파 검사상 자궁내막의 두께가 4 mm 이상인 경우 자궁내막암에 대한 민감도와 특이도는 75%와 40.6%였으며, 자궁내막암, 폴립 그리고 내막증식증과 같은 기질적 문제에 대한 민감도와 특이도는 83.3%와 64.7%였다.
