단태 임신과 쌍태 임신에서 만삭전 조기양막파열의 임상 경과

초록

=목적: 이 연구의 목적은 만삭전 조기양막파열된 쌍태아와 단태아의 잠복기 등의 임상경과를 고찰하고 주산기 예후를 비교하는데 있다. 연구 대상 및 방법: 본 연구는 1996년 1월부터 2000년 12월까지 임신 24주 0일에서 임신 33주 6일 사이에 만삭전 조기양막파열로 본원 산부인과에 입원한 단태 임산부 345명, 쌍태 임산부 73명을 대상으로 후향적 고찰을 시행하였다. 연구 대상 산모는 단태와 다태 임신으로 나누었고 각각 30주 이전과 이후 양막파열군으로 세분하여 잠복기 및 주산기 예후를 비교하였다.결과: 만삭전 조기양막파열된 단태 임신군과 다태 임신군의 양막파열된 주수, 산모의 나이, 자궁수축억제제, 스테로이드 사용, 예방적 항생제 사용 등은 유의한 차이가 없었다. 단태 임신군에 비해 쌍태 임신군의 잠복기가 의미 있게 짧았고 (중앙값 [범위]; 4.0 [0-50] vs. 1.0 [0-25] days, p<0.001), 더 많은 임산부가 48시간 이내와 7일 이내 분만을 하였다 (48시간 이내, 33.0% vs. 65.8%, p<0.001; 7일 이내, 67.5% vs. 87.7%, p<0.001). 두 군 모두에서 30주 이전에 양막파열된 경우가 30주 이후에 양막파열된 경우보다 잠보기가 더 길었다 (중앙값 [범위]; 단태아 11.5 [0-50] vs. 3.0 [0-33] days, p< 0.001; 쌍태아, 3.0 [0-25] days vs. 0 [0-6] day, p<0.001). 두 군에서 분만 시 임신 주수는 유의한 차이가 없었지만 분만 후 쌍태 임신군에서 낮은 출생 체중, 낮은 Apgar score, 높은 신생아 이환율을 보였다.
