쌍태임신에서 임신 제2삼분기에 시행된 맞춤형 선택적 유산에 관한 연구

=목적: 최근 생식 보조술의 증가로 고령 임신 및 다태 임신이 늘어나면서 다태 임신 중 이상 태아가 발견되는 확률이 늘어나고 있다. 임신 2삼분기에 발견되는 태아 이상에 대한 선택적 유산술에 대해 알아보고자 하였다. 연구방법: 1998년 3월부터 2006년 3월까지 이대 목동 병원 및 을지 대학 병원에서 일측 태아의 기형을 동반한 쌍태임신 6예를 대상으로 임신 중반기 선택적 유산을 시행하였다. 이 연구는 여섯 예의 선택적 유산에 대하여 후향적으로 실시되었고, 선택적 유산의 적응증으로는 3예의 구조적 기형 (anencephaly, encephalocele, body stalk anomaly), 1예의 염색체 이상 (21 trisomy) 및 2예의 단일 융모막성 쌍태 임신의 합병증 (acardiac twin, twin to twin transfusion syndrome)이 있었다. 결과: 시술은 각기 융모막성을 고려하여 이중융모막을 가진 4예에서는 KCL injection, 단일 융모막성을 가진 2예에서 각각 alcohol ablation, radiofrequency cord ablation을 시행하였으며 6예 모두에서 시술이 성공적으로 이루어졌고 태아는 모두 제3삼분기에 분만하였다.
