통일 이후 북한 주민의 정신건강 법·정책 제언: ‘외상 후 울분장애(PTED)’와 ‘아노믹 우울’을 중심으로

초록

통일 후 북한의 체제 전환은 혼란과 불안정으로 아노미 상태와 사회적 격변을 초래할 수 있으며 이 과정에서 북한 주민들은 사회적 불평등, 차별을 경험하게 될 가능성이 크다. 이러한 요소는 ‘외상 후 울분장애(PTED)’와 ‘아노믹 우울’을 악화시키는 주요 원인이 될 수 있다. 본 연구는 통일 후 북한 주민들의 정신건강에 영향을 미치는 사회적 결정요인에 주목하며, 이에 대한 법･정책적 대안을 제안한다. 특히 문화적응 이론을 바탕으로 정신건강 취약 집단 및 개인의 특성을 분석하고, 정신건강의 국제기준인 ‘회복 패러다임’을 지향점으로 삼는다. 이를 바탕으로 현재부터 통일 이후의 시점까지 전반적인 법･정책 방향성을 설정한다. 독일이 통일 과정에서 겪었던 경험을 타산지석 삼아 정신적으로 안정된 통일을 이루는 것이 우리의 시대적 과제이다.

Abstract

North Korea’s regime change after unification could lead to confusion and instability, potentially resulting in a state of anomie and social upheaval. During this process, North Korean residents are likely to experience social inequality and discrimination. These factors can be major contributors to the development of posttraumatic embitterment disorder (PTED) and anomic depression. This study focuses on how social determinants of health affect the mental health of North Korean residents after unification and proposes legal and policy recommendations. In particular, it analyzes the characteristics of groups and individuals vulnerable to mental health issues based on acculturation theory, and adopts the international mental health standard, the ‘recovery paradigm,’ as its goal. Based on this, it establishes the overall direction of legal policy from the present to the post-unification period. Achieving a mentally stable unification by drawing lessons from Germany’s reunification experience is our task.
