치매노인 작업치료의 접근과 수행에 영향을 주는 요소

본 연구는 치매노인에게 작업치료의 접근과 수행시 영향을 미치는 변인들을 규명하고자 실시하였다. 이를 위해 문헌연구와 작업치료사의 면담을 토대로 문항을 개발하는 과정을 거친 후 예비조사를 거쳐 치매노인의 작업치료 시 고려되어지는 변인에 관한 최종문항을 개발하였다. 개발한 문항을 이용하여 작업치료사 166명을 대상으로 자료를 수집하였고 자료에 대해 탐색적 요인분석과 상관관계 분석을 실시하였다. 그 결과 치매노인을 위한 작업치료 접근과 수행에서 작업치료사들의 결정에 영향을 주고 있는 요인은 8개, 32문항으로 나타났다. 이들 요인들은 자원과 제반여건, 전문가, 전문지식, 보호자, 생활적응치료, 근거기반치료, 퇴행예방치료, 기능 유지치료로 각각 명명하였다. 작업치료사들이 치매작업치료를 수행할 때 치료사의 개인적 역량과 판단 외에도 환자의 배경과 사회적 여건이 함께 고려되어지는 것으로 보인다. 근무지역, 임상 경력, 직장유형이 다소 영향을 주고 있었다. 따라서, 치매노인을 위한 작업치료의 질적 향상을 위하여서는 치료사들에게 직접적인 영향을 주는 전문지식의 교육과 보수의 적절성이 이루어져야 할 것이며 치료중재에 대한 근거제시와 정책적 지원을 통한 제반여건 확립도 동시에 이루어져야 하는 것으로 보인다.
Abstract
The purpose of this study was to investigate factors that influence the accessibility and performance of occupational therapy for elderly patients with dementia. To this end, a questionnaire was first drafted based on a literature review and interviews with occupational therapists and further developed through a pilot test to explore factors that are taken into consideration when providing occupational therapy to elderly patients with dementia. The questionnaire was used to collect data from 166 occupational therapists, and exploratory factor analysis and correlation analysis were performed on the data. According to the analysis result, 8 factors and 32 questions were found to influence the decisions of occupational therapists with regard to the accessibility and performance of occupational therapy for elderly patients with dementia. The factors were labeled ‘resource’, ‘condition and background’, ‘professional background’, ‘expert information’, ‘guardian’s background’, ‘life adaptation therapy’, ‘evidence-based treatment’, ‘retrogression prevention program’, and ‘maintenance therapy’, respectively. It appears that an occupational therapist takes into account a patient’s background and social conditions on top of his/her individual capabilities and judgment when providing occupational therapy for elderly patients with dementia. Therefore, in order to improve the quality of occupational therapy for elderly patients with dementia, it seems that policy support, including medical insurance, should be provided along professional training and appropriate remuneration which have direct impact on therapists.
