인지-의사소통장애 간편검사(BCCD) 개발을 위한 예비연구

초록

인지-의사소통 장애(Cognitive Communication Disorders)는 인지적 결함에 근거한 의사소통의 문제이다. 특히 고령화에 따라 급증할 것으로 예상되는 후천적 인지-의사소통장애는 실어증과 같은 신경학적 언어장애와 구별되므로 다른 방식의 진단이 요구된다. 본 연구는 인지-의사소통장애 간편검사(Brief test of Cognitive-Communication Disorders, BCCD)를 개발하고 표준화하기 위한 예비연구로서, 문항의 적절성을 검토하는 데 목적을 두었다. 이를 위해 정상 장노년층 171명을 연령에 따라 세 집단으로 분류한 후 BCCD의 수행력과 공인타당도를 분석하였다. 연구 결과는 다음과 같다. 첫째, 연령이 낮고 교육연수가 높은 집단일수록 인지 및 의사소통 영역 전반에서 높은 수행력을 보였다. 둘째, BCCD의 모든 하위 영역 및 총점에서 세 집단 간에 유의미한 차이가 있었고, 고차원 인지, 화용, 의사소통 및 BCCD 총점은 모든 두 집단 간의 차이가 유의미하였다. 셋째, 공인타당도를 검증한 결과 K-MMSE의 시공간력, 기억력, 언어, 총점과 전반적으로 높은 상관성을 보였다. 본 결과는 향후 BCCD의 신뢰도와 타당도를 포괄적으로 검증하고, 정상군 규준, 민감도와 특이도 등을 제시하기 위한 토대로서 활용될 것이다.

Abstract

‘Cognitive Communication Disorders (CCD)’ is a term used to describe a set of communication features that result from underlying deficits in cognition. In particular, Acquired Cognitive Communication Disorders (ACCD) will increase rapidly in aged society. Accordingly, we should evaluate them in separate ways as they are different from other neurological language disorders like aphasia. The purpose of this preliminary study was to develop and standardize the Brief test of Cognitive-Communication Disorders (BCCD), and examine its relevance. A total of 171 middle aged and elderly people were divided into three groups by age, and then their performances and concurrent validity were analyzed. There were three main findings. Firstly, younger group with a higher level of education performed better across domains. Secondly, there was a significant difference among three groups in all subtests and total score of BCCD.We especially searched for significant differences between two groups (group Ⅰ-Ⅱ, Ⅰ-Ⅲ, Ⅱ-Ⅲ) in higher-order cognition, pragmatic language, communication total score, and BCCD total score. Lastly, verification of the concurrent validity resulted in a strong correlation with visuospatial ability, memory, language, and total score of K-MMSE. This study is expected to help confirm the overall reliability and validity of BCCD. It will be also used as the basis for providing normative data, sensitivity, specificity, etc.
