Abstract
Examining the cognitive functions involved in the discourse ability in the elderly is important for identifying language impairments related to cognitive-communication disorders and for future intervention. This study is to investigate discourse performance and related cognitive functions in normal adults aged 55 or older. To this end, the scores of the discourse item, which is the sub-item of the Brief test of Cognitive-Communication Disorders (BCCD) is compared by age, and cognitive functions involved in discourse performance are investigated. This study was conducted on 468 normal adults aged 55 years or older. For 162 participants aged 55-64, 146 aged 65-74,and 160 aged ≥75, the total discourse scores, sub-scores of coherence, cohesion, proposition, and pauses, and the scores of cognitive functions of attention, visuospatial ability, memory, organization, reasoning, problem solving, and executive function were compared by age. In addition, the effects of seven cognitive functions on discourse performance were investigated through multiple regression analysis. The relationship between significant cognition factors and discourse sub-scores was investigated using partial correlation analysis. At the age of 65, discourse performance declined. In the 55-64 year old group, organization was related to discourse performance, and in the 65-74 year old group, organization and problem-solving ability were the main factors predicting discourse performance. In the ≥75 years old, the ability of organization and problem-solving, as well as memory, were cognitive functions that influenced discourse performance. The scores of coherence, cohesion and propositions declined from the age of 65, and the ability of grouping information into a series of meanings and associating related information had an important effect on discourse performance of the elderly aged 55 years or older. In addition, for those in 60s or older, strategies for recognizing and solving tasks are essential for discourse performance. For those in 70s or older, the ability to temporarily hold and manage related information is also necessary to conduct discourse tasks.
