경피신경전기자극치료와 테이핑이 뇌성마비인들의 통증감소에 미치는 효과

많은 성인 뇌성마비는 근긴장도와 운동성의 문제로 인하여 만성통증을 가지고 있으나 성인뇌성마비를 위한 의료, 재활에 대한 서비스는 부족하고 연구는 거의 없는 실정이다. 이에 본 연구는 테이핑과 경피신경전기자극치료을 사용하여 성인 뇌성마비의 통증 조절정도를 알아보고자 하였다. 통증을 호소하는 26명의 성인뇌성마비를 대상으로 격자무늬테이핑군(남 12명, 여 1명; 평균연령 28±2.9세)과 경피신경전기자극치료군(남 11명, 여 2명; 평균연령 27.5±2.8세)으로 분류하여 시각적상사척도(Visual Analogue Scale; VAS)와 현재통증지수(Present Pain Invencity; PPI)를 이용하여 통증정도를 측정하였다. 통계처리는 SPSS 12.0을 사용하여 통증의 차이정도, 뇌성마비분류와 통증부위의 상관관계, 테이핑군과 전기치료군간의 유의성을 알아보기 위해 Wilcoxon 부호순위 검정을 하였다. 통계적 유의수준은 5% 수준으로 설정하였다. 테이핑과 경피신경전기자극치료의 적용전과 후의 통증정도는 VAS와 PPI에서 모두 유의성있게 나타났다(p<.05). 특히 테이핑군이 경피신경자극치료군보다 유의도 있게 나타났다. 따라서, 비침습적이며, 언제 어디서든 적용이 쉽고, 부작용이 거의 없는 테이핑 요법은 성인뇌성마비의 만성통증을 조절을 위한 유용한 방법이라고 하겠다.
Abstract
The purpose of this study was to compare the effects of transcu taneous electrical nerve stimulation and stripes tapping on pain control of adults with Cerebral Palsy(CP). Adults with CP have chronic pain, because they are have problem of mobility, activity and abnormal postural tonus. But which are lack of medical, rehabilitation services and study. Adults with CP was classified into 2 groups, which are transcu taneous electrical nerve stimulation (TENS) group(n=13), Stripe Taping group(n=13). The standard method of cutaneous stimulation, TENS with impulse frequency of 100 HZ, was applied. Their subjective pain before and after the treatment was measured using Visual Analogue Scale (VAS) and Present Pain Invencity(PPI). The results of this study were presented as follows: VAS and PPI score was significantly decreased after TENS and Taping treatment(P<0.05), however, it was more significantly after Taping treatment(P<0.05). In conclusion, this study showed that Taping program and TENS are effective treatment strategy on decrease of Pain. Therefore, It could be considered as a treatment method in the adults with CP and pain control.
