외상성 뇌손상 환자에서 관리기능의 차별적 결손

초록

본 연구의 목적은 외상성 뇌손상 환자에서 관리기능의 결손이 다른 인지기능의 결손 보다 심하다는 가설을 검증하는 것이었다. 이를 위해 정상군 50명과 외상성 뇌손상군 50명을 대상으로 한국판 웩슬러 성인용 지능검사 4판(Korean Wechsler Adult Intelligence Scale-Ⅳ: K-WAIS-Ⅳ), Kims 전두엽-관리기능 신경심리검사 Ⅱ를 실시하였다. 정상군과 외상성 뇌손상군을 관리기능 의존도가 높은 과제와 낮은 과제로 구성한 7개의 대응과제를 사용하여 비교하였으며, 결과는 다음과 같았다. 첫째, 집단(정상군, 외상성 뇌손상군)과 대응점수(관리기능 의존도 높음, 관리기능 의존도 낮음)의 상호작용이 유의하였다. 둘째, 정상군에 비해 외상성 뇌손상군의 수행이 낮은 정도는 관리기능의존도가 낮은 과제에 비해 높은 과제에서 보다 심하였다. 셋째, 정상군은 대부분 유의한 차이가 없는데 반해 외상성 뇌손상군에서 관리기능 의존도가 낮은 과제에 비해 높은 과제의 수행이 유의하게 낮았다. 이러한 결과들은 외상성 뇌손상 환자에서 관리기능이 다른 인지기능에 비해 보다 심하게 결손되어 있음을 제시한다.

Abstract

The purpose of this study was to test the hypothesis that TBI patients are associated with a greater deficit in executive function in relation to other cognitive functions. To this end, 50 TBI patients and 50 normal subjects were compared on Korean Wechsler Adult Intelligence Scale-Ⅳ(K-WAIS-Ⅳ), Kims Frontal-Executive Function Neuropsychology Test Ⅱ. We compared the performance of TBI patients and normal subjects on seven paired tests on which one test strongly engaged executive functions (high EF tests) and the other test weakly engaged executive functions(low EF tests). Results were as follows. First, There were significant interactions between group (normal, TBI) and paired score (involvement of EF high, low). Second, Cognitive deficits of TBI patients relative to normal subjects were greater in high EF test compared with low EF tests. Third, Normal subjects showed no significant difference, whereas TBI patients showed a significantly lower performance in high versus low EF tests. These results are consistent with the hypothesis that TBI patients are associated with a differential deficit in the executive function in relation to other cognitive functions.
