(Timed Up & Go test, TUG), 10m보행 검사(10m walk test, 10mWT), 6분 보행 검사(6Minute Walk Test, 6MWT), 버그균형 척도(Berg Balance Scale, BBS), 체간 장애 척도(Trunk Impairment Scale, TIS), Functional In Sitting Test(FIST), 퓨글 마이어 하지 기능(Fugl-Meyer Lower/Extremity, FM-L/E)를 평가한 데이터를 수집하여 분석하였다.
Abstract
This study aimed to determine the discriminative and predictive validity of initial functional performance evaluation variables for predicting limited community ambulation in stroke patients following robot-assisted gait training. A retrospective study was conducted on 41 stroke patients who received inpatient treatment and underwent robot-assisted gait training at the National Rehabilitation Center from January 2019 to December 2021. Data were collected and analyzed from functional performance evaluations before and after training—including the Functional Ambulation Category (FAC), Timed Up & Go test (TUG), 10m Walk Test (10mWT), 6-Minute Walk Test (6MWT), Berg Balance Scale (BBS), Trunk Impairment Scale (TIS), Functional In Sitting Test (FIST), and Fugl-Meyer Lower Extremity (FM-L/E). The cutoff values for pre-training functional performance evaluations were established to predict post-training walking ability [<0.4㎧=Household walker, 0.4-0.8㎧= limited community walker]. Discriminative and predictive validity were determined using the odds ratio. The initial BBS cutoff value for predicting limited community walker after robot-assisted gait training was 39.5 points, with an AUC of 0.713 and moderate accuracy (p=0.025). Patients with an initial BBS >39.5 points before training were 0.156 times more likely to predict limited community walker compared to those with BBS ≤39.5 points. An initial BBS >39.5 points in stroke patients is a discriminative variable for predicting limited community walker (mean 0.6 m/s; range 0.41-0.98 m/s) after robot-assisted gait training, with an accuracy of 68%. The benefits of robot-assisted gait training were more significant in balance recovery than in walking speed improvement. Thus, dynamic balance recovery should precede improvements in walking ability.
