Abstract

Neonatal alloimmune thrombocytopenia (NAIT) is caused by an immunological reaction between a mother’s alloantibodies and antigens expressed on the fetus’s platelets. Human platelet antibodies (HPA) and human leukocyte antigens (HLA) class I are known to be associated with this condi- tion. The majority of cases are attributed to reactions driven by the specific antigenicity of these antibodies. However, cases attributed to cross-re- activity against shared epitopes have not been documented. We describe a rare case of NAIT associated with HLA cross-reactive group (CREG) antibodies. A newborn exhibited severe thrombocytopenia (platelet count of 81×10 /L) immediately after birth. Laboratory tests detected anti- 9 HLA class I antibodies that belonged to the same HLA CREG (HLA A2 CREG, Bw6 CREG) as the newborn’s HLA typing (A11, A24, B61). The platelet count recovered after the administration of Intravenous immunoglobulin (IVIG) and platelet transfusion. These findings suggest that the HLA CREG antibodies were associated with the neonate’s thrombocytopenia and may have significant clinical implications.
